Waiver & Assumption of Risk

All participants must have a signed Waiver and Assumption of Risk Agreement on file before participating in any Michigan Sports Performance (MSP) training session, clinic, camp, evaluation, or event. Participants under the age of 18 must have the form completed and signed by a parent or legal guardian.

Please complete the form below in its entirety. Registration is not complete until the waiver has been submitted and accepted.

By signing this agreement, you acknowledge that participation in athletic activities involves inherent risks, including the risk of serious injury. You voluntarily assume these risks and agree to release Michigan Sports Performance Company LLC (MSP), its owners, coaches, trainers, staff, volunteers, affiliates, and facility partners from liability for injuries or damages arising from participation, except as prohibited by law.

LIABILITY WAIVER *

Assumption of Risk I understand that participation in Michigan Sports Performance (MSP) training sessions, camps, clinics, evaluations, strength and conditioning, speed and agility training, basketball instruction, volleyball instruction, team training, and any other athletic activities involves inherent risks. These risks include, but are not limited to: Falls, Muscle strains, and sprains, Broken bones, Concussions, Heat-related illness, Cardiac events, Permanent disability, Death. I voluntarily choose to participate (or allow my child to participate) knowing these risks exist. I certify that I (or my child) am physically able to participate in athletic activities and have no medical condition that would prevent safe participation. I understand that it is my responsibility to consult with a physician before participating if there are any concerns regarding health or physical condition. In consideration for being allowed to participate in any MSP program, I, on behalf of myself, my child, my heirs, executors, administrators, and assigns, hereby release and forever discharge: Michigan Sports Performance Company LLC (MSP), Coach Kris Daiek, coaches, trainers, employees, volunteers, contractors, facility owners, landlords, affiliates, sponsors, and representatives from any and all liability, claims, demands, actions, causes of action, damages, costs, expenses, attorney fees, or losses arising out of or related to participation in any MSP activity, including those arising from ordinary negligence. Emergency Medical Treatment. If I am unable to be contacted in the event of an emergency. I authorize MSP staff to obtain emergency medical treatment for myself or my child. I understand that all medical expenses are my sole responsibility. Insurance. I understand that MSP does not provide medical, accident, or health insurance for participants. I certify that I have adequate medical insurance coverage or accept full financial responsibility for any medical expenses incurred. I agree to: Follow all instructions given by coaches and staff. Use equipment safely. Conduct myself in a respectful and sportsmanlike manner. Notify coaches immediately of any injury or medical concern. Failure to follow safety instructions may result in removal from participation without refund. I understand there is an inherent risk of exposure to contagious illnesses, including but not limited to COVID-19, influenza, and other communicable diseases. I voluntarily assume these risks. If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect. This Agreement shall be governed by and interpreted under the laws of the State of Michigan.
I have carefully read this Waiver and Release of Liability. I understand its contents. I understand I am giving up substantial legal rights. I sign this Agreement voluntarily and without coercion. By submitting this form I agree to all terms and conditions.
